Washington Alcohol Server Permits
Replace Lost or Stolen Permits (includes changing your name)
Order Online: If we trained you, we can replace lost or stolen permits or issue a new permit if you have changed your name. The fee for this service is $6.00.
We do not offer an order by mail option.

Upgrade your Permit to Class 12
Order Online: If we trained you, we can upgrade your permit from a Class 13 to a Class 12 after you have turned 21. The fee for this service is $6.00.

Resend / Recreate your Permit
If you do not recall getting your permit and it has been:
Less than 2 months from course completion, call 360-458-2240 to discuss the situation. If you have to leave a message, make sure you leave your Last Name, Phone #, and Date of Birth so we can look you up before we call back.
Greater than 2 months from course completion. Go to the Permit Checker and with your Last Name and Date of Birth find out if your permit was reported to the Liquor Board. Then call 360-458-2240 to discuss the situation. Have your Permit # and class date available.
If your permit was returned by the Post Office, we make every effort to contact you to resolve the problem. If the situation is not resolved within 2 months of the date we mailed your permit, we will destroy the permit and it will be treated as a "Lost Permit."
FYI, our Lost Permit fee is $6 and our Resend Permit fee is $4.
